HCC’s Re-Entry hub aims to demonstrate the impact of support services on recidivism rates and is based on national models for reducing crime.

Participants are connected to services that provide clear, step-by-step guidance on disclosing their criminal background on job applications or during interviews and navigating other common challenges related to employment, housing, and education post release. Through essential resources and a strong network of support services, HCC is helping individuals reentering society build a new path forward and embrace a second chance at life.

The Re-Entry Hub is offered by HCC Network. If you are in need of Medical, Dental or Behavioral Health services, HCC Network clinics are here for you.
General Health and Primary Care:
  • Cold, flu, or COVID-like symptoms (e.g., cough, fever, sore throat)

  • Allergies or sinus infections

  • Rashes or skin conditions (e.g., eczema, acne, bug bites)

  • Minor injuries (e.g., sprains, cuts, burns)
