Project Connect 

Service providers and volunteers are needed for upcoming Project Connect events. To learn more and get involved, contact Suzanne Smith.

 
Resources available at Project Connect events include:
  • Dental extractions in our mobile unit (first come, first serve).
  • Hair cuts.
  • Height, weight, blood pressure check, vision screenings, etc.
  • Behavioral health screenings.
  • Enrollment assistance for Medicaid, Medicare, Affordable Care Act, unemployment, disability, and food stamps.
  • WIC & nutritional information & education.
  • Pregnancy tests & women’s resources.
  • Exit bags with hygiene supplies & necessities.
  • Workforce mobile unit.
  • Veteran’s services.
  • Immunizations voucher (good for 14 days).
  • Birth/death certificate voucher (good for 14 days).
  • MO driver’s license & photo ID voucher (good for 14 days).
  • Many other resources – see below!
We are looking for vendors and volunteers for upcoming Project Connect events throughout the year. Organizations will help raise awareness of available community resources by providing information and free services while also building connections with members of the community. Volunteers will guide attendees around the event and inform them about the community resources and services available. We are also in need of sponsorships to help provide event services as well as meals for Project Connect guests, providers, and volunteers. Contact Suzanne for more information.

Project Connect events are organized by HCC Network, its Community Health Centers, and the Social Service Committee.

Our community-based programs include case management, counseling and assessment, health education, transportation, translation services, enrollment assistance, health and social care advocacy initiatives, access to primary care, and health IT.

After more than a decade and nearly 50 new employees later, it’s safe to say that the Lexington, MO-based HCC Network is a major player in Lafayette County and in rural health care in this region.

The focus of HCC is to develop and implement programs that are responsive to documented health needs of county residents, with specific health status indicators as benchmarks of progress.
