This initiative, established by the Department of Veterans Affairs (VA), ensures that veterans have timely access to high-quality healthcare services within their communities.

Understanding the Veterans Community Care Network

The CCN serves as a bridge between the VA and community healthcare providers, facilitating seamless care for eligible veterans. By integrating community providers into the VA’s healthcare system, the CCN addresses challenges such as long wait times and travel distances, ensuring veterans receive the care they need without unnecessary delays.

HCC Network’s Role in the CCN

As a designated CCN provider, HCC Network offers a range of medical services to veterans across our clinics in Buckner, Carrollton, Concordia, Lexington, and Waverly. Our commitment is to deliver comprehensive, patient-centered care that respects the unique experiences and needs of each veteran.

Steps for Veterans to Access Care at HCC Network Clinics

1. Verify Clinic Availability: Ensure that one of our clinic locations aligns with your healthcare needs.

2. Obtain a VA Referral: Contact your local VA Medical Center to request a referral to HCC Network.

3. Eligibility Determination: The VA will assess your eligibility based on specific medical requirements and geographic considerations. If approved, the referral is forwarded to the Third-Party Administrator managing the CCN in your region.

4. Schedule an Appointment: Once authorized, schedule your appointment with an HCC Network provider. Assistance with scheduling may be available through the Third-Party Administrator.

5. Confirm Authorization: At your appointment, verify that the care is VA-authorized to ensure billing is processed through the VA, preventing out-of-pocket expenses.

6. Continuity of Care: Post-appointment, HCC Network will transmit your medical records to the VA, maintaining an up-to-date health profile.
