HCC Network Clinics are Part of the VA’s Community Care Network

You’ve served us well. Now, it’s our turn.

HCC Network’s clinics have received designation as a Veteran’s Community Care Network provider. The VA Community Care Network (CCN) is the VA’s direct link with community providers to ensure Veterans receive timely, high-quality care. CCN uses industry-standard approaches and guidelines to administer services, pay for services promptly, and manage the network to its full potential.

HCC can provide community care for Veterans through the Mission Act. 

How Can I Get an Appointment? How Does Using the VA’s Community Care Network work?

    1. Verify one of our clinic locations will work for your needs. See the VA provider map here.

    2. Ask for a referral from your local VA Medical Center

    3. Your eligibility is determined by the VA based on your specific medical needs and location. If you are eligible, the VA will process your request and send the referral to the Third-Party Administrator managing the Community Care Network in your region.

    4. Once approved, you can schedule an appointment with a VA-authorized community provider. The Third-Party Administrator may assist with scheduling.

    5. At your appointment, verify that the care is authorized by the VA and will be billed through them, not directly to you.

    6. After your appointment, your HCC provider will send your medical records back to the VA to update your file.
