Family Nurse Practitioner – Concordia

Fawn Zimmer graduated with her nursing degree in 2011 and began working as an RN in multiple intensive care units and emergency departments across the U.S. She graduated with her master’s degree from Graceland University in 2015. She has worked as a Family Nurse Practitioner in both the emergency department and family practice settings, and taught nurse practitioner students in the Graduate setting. Zimmer was previously employed with HCC at the Waverly clinic in 2016 prior to moving out of state with her husband, an active-duty pilot in the U.S. Air Force. They have recently moved back to Missouri and she has rejoined the Community Health Center in Concordia. Zimmer and her husband Drew have a two-year-old son and recently added a baby girl to their family. Zimmer said she is excited to be back in Missouri seeing patients of all ages in a rural practice setting.

Our community-based programs include case management, counseling and assessment, health education, transportation, translation services, enrollment assistance, health and social care advocacy initiatives, access to primary care, and health IT.

After more than a decade and nearly 50 new employees later, it’s safe to say that the Lexington, MO-based HCC Network is a major player in Lafayette County and in rural health care in this region.
