HCC RECEIVES $1MILLION IN GRANT FUNDING FOR TRANSITIONS IN CARE,
POST-INCARCERATION PROJECT

Lexington, Mo, December 2, 2024 – HCC Network was awarded $1,000,000 in total for two years to support the work of HCC Transitions in Care for a justice-involved populations project. The project will focus on strengthening re-entry care for individuals who will soon be released from incarceration by connecting them to comprehensive, integrated healthcare and support services.

“It is our goal to provide a safe environment for recently released individuals to be able to apply for jobs, talk to a Peer Coach or Community Health Worker, apply for Medicaid, etc. Inmates will be assigned an individual to work with pre-release and throughout the remainder of their incarceration to have a seamless transition back into society,” shares Kayla Schmidt, Director of Operations.

Project Director and Substance Use Coordinator, Molly Adams said, “Our post-incarceration population is historically underserved and stigmatized. I am grateful that HCC Network has identified the need to bring resources to these individuals. I believe these resources, along with community support, will have a major impact on these individuals.”

The HCC Transitions in Care Project will include pre-release planning, a mobile community resource hub, a community harm reduction vending machine and referrals to support services, such as MAT, peer support, care coordination, primary care services, behavioral health services, dental services and  screenings for social determinants of health.

This program will allow HCC to:

  • Support mental health and substance use disorders and reduce the risk of drug overdose through our Peer Support Specialists
  • Address health-related social needs such as food and housing insecurity, financial strain and transportation barriers through our Community Health Workers
  • Provide chronic disease management through our RN Patient Care Coordinators

HCC Network plans to work diligently with community partners to ensure a seamless transition with the overall goal of decreasing recidivism rates in our rural community.

This grant funding was part of a larger award from the Health Resources and Services Administration (HRSA), an agency of the U.S. Department of Health and Human Services (HHCS) of nearly $52 million in awards for HRSA funded health centers to increase access to high-quality primary care for individuals soon to be released from incarceration and reentering the community. HCC received support from Community Asset Builders, LLC in writing this grant.
