Breann is the Administrator of the Lafayette County Health Department. Breann obtained her bachelor’s degree in Health Science with an emphasis on Community Health from the University of Central Missouri.  Breann has worked at the health department for over six years. During the first five years with the department, she led a community-wide health needs assessment engaging many community partners across the county. She also served as the Community Health Educator managing multiple programs across the county. These programs included chronic disease management among adults and child wellness programs within the school districts of Lafayette County. Breann lives in Higginsville, MO with her husband Nick and their two children.

Our community-based programs include case management, counseling and assessment, health education, transportation, translation services, enrollment assistance, health and social care advocacy initiatives, access to primary care, and health IT.

After more than a decade and nearly 50 new employees later, it’s safe to say that the Lexington, MO-based HCC Network is a major player in Lafayette County and in rural health care in this region.

The focus of HCC is to develop and implement programs that are responsive to documented health needs of county residents, with specific health status indicators as benchmarks of progress.
