About Us
HCC Network was founded in 2004 under the name Health Care Coalition of Lafayette County. We quickly established partnerships with local health centers to grow our community impact and now serve over 7,900 unique patients in west central rural Missouri through our network of six clinics which offer medical, dental and behavioral health services. As a Federally Qualified Health Center (FQHC), we provide primary care services to underserved populations, with many in our service area falling well below the poverty line. As a community health center we serve all individuals, regardless of their ability to pay including Medicare, Medicaid, insured and offer a sliding scale fee schedule for uninsured individuals.
Beyond our physical exams, dental care and behavioral health services, we can help patients navigate addiction recovery through our Peer Support Specialists, offer financial assistance and state and local resource programs including WIC, SNAP (Food Stamps) and LIHEAP via our Community Health Workers, provide chronic condition management through our RN Patient Care Coordinators. Transportation services, mobile medical and dental units, community outreach, monthly food box programs, resource warehouse and 340B Prescription Drug Program are some of the additional ways HCC Network brings value to our community.
